By registering on a course:

 I give consent for my child to take part in Roehampton and Fulham Juniors Cricket Programmes.

 I give my consent that if an emergency medical situation arises, Roehampton and Fulham Juniors may act as in loco parentis. If the need arises for administration of first aid and/or other medical treatment which in the opinion of a qualified medical practitioner may be necessary. I also understand that in such circumstances that all reasonable steps are made.

 I can confirm that my child is aware of Roehampton and Fulham Juniors code of conduct for young people and anti-bullying policy.

 I understand that my child must wear a suitable helmet when batting, standing up to the stumps when keeping wicket or fielding in certain positions, along with other required safety protection relevant to the role.

 I understand that I may be charged an admin fee of 5% for refunds to cover the transaction fees levied by the booking system provider

 I understand that I can request a Refund or Credit for a future booking for a session cancelled by Roehampton and Fulham Juniors

 I understand that I need to notify Roehampton and Fulham Juniors if I wish to receive a Refund or Credit for a session my child missed due to a medical emergency or having to isolate due to government guidance. I will endeavour to make that notification prior to the session start

 I understand that it may be possible to take make-up sessions within the same session term for absences notified to Roehampton and Fulham Juniors at least 48 hours in advance of the session start time for reasons other than those outlined above at the discretion of Roehampton and Fulham Juniors

 I understand that all credits expire one year after the the date of the session they were granted for

I confirm that I have read, or been made aware of, the organisation’s policies concerning:

 Safeguarding

 Codes for conduct for parents, coaches, & young people

 Transport policy

 Changing room policy

 Photography, videoing, texting and use of social media policies.
